Roles & Responsibilities

• Provide compassionate direct patient care including bathing, grooming and oral hygiene
• Assist patients with toileting, positioning, transferring and mobility to promote safety and dignity
• Measure and record vital signs, intake and output, height and weight and report abnormalities
• Collect specimens such as stool, urine and sputum accurately for analysis
• Administer treatments under nurse direction including enemas and oxygen via cannula or mask
• Feed patients and prepare or pass meals according to diet charts
• Maintain accurate, timely documentation on hospital-approved flow sheets
• Ensure infection control and a clean, safe patient environment
• Participate in recreational and therapeutic activities as assigned to support patient well-being
• Assist with admissions, transfers and discharges and safeguard patient personal property

Qualifications

• Current CNA credential or equivalent certification as required by facility
• Experience with ADLs, vital signs, specimen collection and documentation
• Comfortable performing patient lifting and transfers using proper equipment
• Familiarity with infection control procedures and safe patient care practices
• Experience or willingness to work in long-term care, medical-surgical, SNF or ICF settings
• Ability to work rotating shifts including evenings
• Willingness to participate in in-service education and follow facility policies

Knowledge & Skills

• Strong patient care skills including bathing, feeding and toileting assistance
• Proficient in taking and recording vital signs and intake and output
• Skilled in specimen collection and proper documentation
• Knowledge of infection control and sterilization procedures
• Able to operate patient care equipment and assist with transfers safely
• Comfortable supporting recreational activities and contributing to care plans
• Good communication with patients, families and nursing staff
