Roles & Responsibilities

• Assess and interpret patient evaluations to create individualized PT treatment plans
• Deliver hands-on manual therapy, therapeutic exercises, and assistive device training
• Administer physical agents and modalities including hydrotherapy, ultrasound, traction and moist heat
• Design and teach home exercise programs to ensure continuity of care after discharge
• Evaluate prosthetic and orthotic fit and recommend adjustments for optimal function
• Document patient care thoroughly in department and patient records
• Collaborate with physicians, nurses, social workers and care teams for effective discharge planning
• Maintain equipment, troubleshoot issues, and ensure a safe, clean treatment environment
• Lead staff development through in-service training and professional mentoring
• Adhere to infection control, legal, and professional standards to protect patients and staff

Qualifications

• Current Physical Therapy license issued by the State of Colorado
• BLS (Basic Life Support) certification required
• Comfortable treating patients across the lifespan — pediatric through geriatric
• Experience with inpatient rehab, swing bed or skilled nursing populations is a strong fit
• Strong documentation skills and ability to collaborate with interdisciplinary teams
• Ability to safely perform manual therapy, transfer training, and assistive device instruction

Education

• Physical Therapy licensure required (State of Colorado). Specific degree not stated in posting.

Knowledge & Skills

• Patient assessment and treatment planning with measurable functional goals
• Manual therapy techniques and therapeutic exercise progression
• Hydrotherapy, ultrasound, traction, prosthetics and orthotics familiarity
• Home exercise program design and patient/family education
• Infection control practices and safe patient handling
• Experience leading in-service trainings and contributing to staff development
• Equipment maintenance basics and following manufacturer guidelines
• Strong communication, documentation, and teamwork skills
