Roles & Responsibilities

• Provide compassionate bedside care to residents and patients in a rehab setting
• Administer medications and treatments safely and accurately
• Monitor patient condition, record vital signs, and report changes promptly
• Collaborate with nurses, therapists, and the care team to coordinate excellent care
• Educate patients and families about care plans and discharge instructions
• Maintain accurate documentation in the medical record and follow facility protocols
• Adhere to infection control, safety, and compliance standards at all times

Qualifications

• Current and unencumbered Licensed Practical Nurse (LPN) license in New Mexico
• Current BLS/CPR certification or willingness to obtain prior to start
• Comfortable working 12+ hour night shifts and flexible per diem scheduling
• Strong communication skills and a team-first attitude
• Ability to pass required background checks and drug screening
• Experience in long-term care, rehabilitation, or similar clinical settings is preferred

Education

• Graduate of an accredited Practical Nursing program or equivalent vocational nursing diploma

Knowledge & Skills

• Proficient in medication administration and basic clinical procedures
• Skilled at patient assessment, vital signs, and monitoring
• Experience with electronic health record documentation
• Knowledge of wound care basics and mobility assistance
• Excellent time management, prioritization, and teamwork skills
• Clear communication with patients, families, and interdisciplinary teams
