Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a rehab setting
- Administer medications and treatments accurately and safely
- Assess patient status, document changes, and communicate effectively with the care team
- Assist with ADLs and support residents’ mobility and comfort
- Collaborate with RNs, therapists, and support staff to implement care plans
- Maintain accurate charts and complete timely clinical documentation
- Support infection control practices and maintain a safe environment for residents
Qualifications
- Current Licensed Practical Nurse (LPN) license in New Mexico or compact-state equivalent
- Basic Life Support (BLS/CPR) certification (current)
- Experience in long-term care or rehabilitation settings preferred
- Strong clinical judgment, ability to manage multiple priorities during night shifts
- Clear background checks and required facility onboarding completed
Education
- LPN diploma, certificate, or vocational nursing program completion
- Current LPN licensure required
Knowledge & Skills
- Medication administration and monitoring
- Wound care basics and dressing changes
- Resident assessment and vital signs monitoring
- Electronic medical record (EMR) charting and documentation
- Effective communication with patients, families, and interdisciplinary teams
- Familiarity with rehab-focused care and safety protocols
