Roles & Responsibilities
- Provide compassionate direct patient care to residents in the rehab center
- Administer medications and document medication administration per policy
- Monitor patient condition, record vital signs, and report changes promptly
- Collaborate with nurses, therapists, and the interdisciplinary team to create great care plans
- Document accurate, timely charting in the electronic health record
- Assist with admissions, discharges, transfers, and hands-on nursing procedures
- Maintain infection control, safety standards, and a clean therapeutic environment
Qualifications
- Current LPN license in New Mexico required
- Current Basic Life Support (BLS) certification preferred
- Completion of an accredited Practical Nursing program
- Comfort working overnight shifts in a rehab/long-term care environment
- Strong communication skills and ability to work well on a collaborative team
- Immunizations and health screenings per facility policy
Education
- Graduate of an accredited Practical Nursing program and active LPN licensure
Knowledge & Skills
- Skilled in medication administration and resident safety
- Solid clinical assessment and documentation abilities
- Familiarity with rehab/long-term care workflows and patient populations
- Good time management, prioritization, and multitasking in busy shifts
- Proficient with electronic health records and charting best practices
- Team player attitude with a focus on patient-centered care
