Roles & Responsibilities

• Provide safe, compassionate nursing care to residents in accordance with care plans
• Administer medications and treatments accurately and document thoroughly
• Perform routine assessments, monitor vital signs, and report changes promptly
• Collaborate with interdisciplinary team members to support resident goals
• Assist with activities of daily living while promoting resident dignity and independence
• Maintain clear, timely charting and communicate effectively with families and staff
• Follow infection control and safety protocols to protect residents and team members

Qualifications

• Current Licensed Practical Nurse (LPN) credential — state licensure or eligibility required
• BLS/CPR certification required or obtained upon hire
• Previous LPN experience in long-term care, rehabilitation, or a related setting preferred
• Strong clinical assessment and medication administration skills
• Excellent communication, teamwork, and time-management abilities
• Ability to pass facility-required background and health screenings

Education

• Graduate of an accredited LPN/Vocational Nursing program or equivalent

Knowledge & Skills

• Medication administration and documentation best practices
• Basic nursing assessments and vital sign monitoring
• Wound care fundamentals and restorative nursing approaches
• Familiarity with electronic health record documentation
• Infection control, safety protocols, and resident-centered care
• Effective communication with residents, families, and care teams
