Roles & Responsibilities

• Provide compassionate overnight nursing care to residents at Bear Canyon Rhb Ctr
• Administer medications and treatments according to physician orders and facility protocols
• Monitor residents’ conditions, document changes, and communicate findings to the care team
• Assist with admissions, transfers, and discharge processes while maintaining dignity and comfort
• Collaborate with interdisciplinary team members to develop and follow individualized care plans
• Support a safe environment by following infection control, safety, and facility policies
• Deliver clear, timely documentation and handoffs to ensure continuity of care

Qualifications

• Active LPN license in New Mexico (or compact/state-appropriate licensure)
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Previous LPN experience in long-term care, rehabilitation, or similar settings preferred
• Strong communication skills and a patient-centered approach
• Ability to work overnight shifts and provide reliable per-diem coverage

Education

• Graduate of an accredited Licensed Practical Nurse program

Knowledge & Skills

• Solid knowledge of medication administration and documentation best practices
• Skilled in basic nursing assessments and vital sign monitoring
• Familiarity with electronic health records and accurate charting
• Comfortable working independently during overnight shifts and escalating concerns appropriately
• Team-oriented mindset with good time management and prioritization skills
