Roles & Responsibilities

• Evaluate patients to develop individualized treatment plans that restore mobility and independence
• Deliver hands-on and therapeutic exercise interventions to improve functional outcomes
• Monitor patient progress and adjust plans to achieve measurable improvements
• Collaborate with nursing and the interdisciplinary team to coordinate comprehensive care
• Educate patients and families on home programs and safety to support long-term recovery
• Document assessments, interventions, and progress clearly and timely in the medical record

Qualifications

• Valid California Physical Therapist license (required)
• Current BLS/CPR certification or the ability to obtain upon hire
• Experience with post-acute, rehab, or skilled nursing populations preferred
• Strong clinical reasoning, assessment, and manual therapy skills
• Excellent communication skills and a team-first attitude

Knowledge & Skills

• Skilled in patient assessment, goal setting, and individualized treatment planning
• Proficient with therapeutic exercise, manual techniques, and mobility training
• Comfortable documenting in electronic health records and meeting regulatory standards
• Ability to work collaboratively with nurses, aides, physicians, and family members
• Strong time management and caseload prioritization skills
