Roles & Responsibilities

• Provide compassionate bedside care to residents consistent with LPN scope of practice
• Administer medications, document doses, and monitor patient responses
• Perform assessments and communicate changes in condition to the care team
• Collaborate with RNs, physicians, and interdisciplinary staff to deliver coordinated care
• Assist with wound care, dressing changes, and ROM as indicated
• Maintain accurate, timely documentation in the medical record
• Support a safe, respectful environment and help promote patient dignity

Qualifications

• Current LPN license (practical/vocational nurse) — New Mexico licensure preferred
• Completion of an accredited practical/vocational nursing program
• Basic Life Support (BLS) certification or willingness to obtain
• Clinical experience in long-term care or rehab settings is a plus
• Strong communication skills and a team-oriented mindset
• Ability to pass facility-required background checks and screenings

Education

• Diploma or certificate from an accredited practical/vocational nursing program

Knowledge & Skills

• Solid fundamentals in medication administration and documentation
• Knowledge of infection control and resident safety practices
• Basic assessment skills and the ability to recognize changes in condition
• Comfortable using electronic charting systems and following care plans
• Good time management, organization, and prioritization on busy shifts
• Professional bedside manner and the ability to de-escalate when needed
