Roles & Responsibilities
- Provide compassionate, patient-centered nursing care as an LPN in long-term care and rehab settings
- Administer medications and document accurately and timely
- Monitor and report changes in patient condition to RN leadership and providers
- Assist with ADLs and support resident mobility and dignity
- Collaborate with interdisciplinary team to implement individualized care plans
- Participate in shift handoffs and maintain clear, concise charting
Qualifications
- Active LPN license (state of practice required or eligible for licensure)
- Current Basic Life Support (BLS) preferred or willingness to obtain
- Previous experience in long-term care or similar clinical setting preferred
- Ability to pass required background checks and health screenings
- Strong communication skills and a team-oriented mindset
Education
- Graduate of an accredited practical nursing (LPN/LVN) program
- Diploma or certificate in Practical Nursing
Knowledge & Skills
- Medication administration and basic clinical assessments
- Charting and electronic health record familiarity
- Infection control practices and safe patient handling
- Time management, prioritization, and reliable follow-through
- Positive bedside manner and family communication skills
