Roles & Responsibilities
- Provide compassionate direct patient care as a Licensed Practical Nurse on assigned shifts
- Administer medications and treatments safely and document in the patient record
- Perform accurate patient assessments and report changes promptly to the care team
- Assist with wound care, dressing changes and other nursing procedures within LPN scope
- Collaborate with RNs, physicians and support staff to coordinate excellent patient care
- Maintain clear, timely documentation and uphold facility policies and compliance standards
- Support patient and family education with clear, empathetic communication
- Contribute to a positive unit culture by mentoring peers and sharing best practices
Qualifications
- Current LPN license (active and in good standing) — required
- Basic Life Support (BLS/CPR) certification preferred or willing to obtain
- Prior LPN clinical experience in long-term care or similar settings preferred
- Ability to work night shifts and adapt to per diem scheduling
- Strong communication, time management and teamwork skills
Education
- Graduate of an accredited practical nursing program or vocational nursing certificate/diploma
Knowledge & Skills
- Medication administration and documentation
- Patient assessment and vital sign monitoring
- Wound care fundamentals and dressing changes
- Electronic health record charting and clinical documentation
- Infection control practices and patient safety
- Effective communication with patients, families and interdisciplinary teams
