Roles & Responsibilities

• Deliver compassionate, patient-centered nursing care to residents at the Bear Canyon Rehab Center
• Administer medications and treatments accurately and document everything clearly
• Monitor and report changes in patient condition to the healthcare team promptly
• Support rehabilitation activities and assist patients with daily living needs
• Collaborate with RNs, therapists, and support staff to create and follow care plans
• Maintain clinical documentation in the facility electronic records with attention to detail
• Promote a safe, clean environment and follow infection control and safety protocols

Qualifications

• Current Licensed Practical Nurse (LPN) license - New Mexico preferred or ability to obtain
• Basic Life Support (BLS) certification required or willing to obtain
• Experience in rehab, long-term care, or med-surg settings is highly valued
• Strong clinical assessment and medication administration skills
• Excellent communication and teamwork skills with patients and families
• Ability to work overnight shifts and adapt to per diem scheduling

Education

• Completion of an accredited LPN/vocational nursing program
• High school diploma or GED

Knowledge & Skills

• Medication administration and nursing documentation best practices
• Familiarity with rehab and long-term care workflows and care plans
• Vital signs monitoring, wound care basics, and patient hygiene assistance
• Comfortable with electronic health record systems and charting
• Prioritization and time management during busy overnight shifts
• Professional bedside manner and family communication skills
