Roles & Responsibilities

• Provide direct patient treatment as directed by the supervising Physical Therapist, delivering hands-on care that makes a measurable difference
• Record daily treatment notes and weekly progress notes in compliance with PT board and facility policies
• Follow physician treatment plans and document changes according to policy and procedures
• Assist with maintenance and cleanliness of the treatment area and equipment to ensure a safe environment
• Participate in patient care and rehabilitation conferences to coordinate excellent multidisciplinary care
• Train and support restorative aides and nursing staff through caregiver training and in-service education
• Communicate patient progress, problems, and plans clearly with supervisors and the health care team
• Report and follow up on any equipment issues to maintain reliable therapeutic tools

Qualifications

• Licensed Physical Therapist Assistant in good standing with the state board (required)
• Proficiency with clinical documentation systems such as Casamba and PointClickCare
• Able to meet ongoing continuing education requirements as per state practice
• Strong communication skills and ability to relate positively with patients, families, and staff
• Comfortable participating in in-service training and mentoring restorative aides
• Willingness to follow established policies, procedures, and treatment plans

Education

• Licensed Physical Therapist Assistant (state licensure required)

Knowledge & Skills

• Skilled in patient treatment, documentation, and developing/implementing treatment plans
• Experienced with progress notes, daily treatment notes, and organized recordkeeping
• Familiarity with Casamba and PointClickCare or similar EMR systems
• Ability to provide caregiver training and instruct families or nursing staff for discharge planning
• Competence in equipment maintenance and reporting issues promptly
• Teamwork-minded with strong interpersonal and communication abilities
