Shift Type: Day shifts (specific start/end times not provided); On-call rotation 1-3 shifts per week (includes 3 weekends during the 13-week contract); must be able to respond within 45 minutes when on-call; holiday availability required.

Candidate Pay Rate: $63 per hour

Roles & Responsibilities
- Perform comprehensive patient assessments and coordinate individualized hospice care plans
- Provide expert pain and symptom management consistent with each patient’s plan of care
- Educate and support patients and families on disease progression, comfort measures, and hospice philosophy
- Collaborate closely with physicians, social workers, chaplains, volunteers and the interdisciplinary team
- Participate in the on-call rotation and respond promptly to patient needs when assigned
- Maintain accurate, timely clinical documentation in compliance with state, federal, and organizational policies
- Deliver compassionate, dignified care in patients’ homes and serve as a trusted clinical resource for families
Qualifications
- Current Registered Nurse (RN) license in Utah or a compact state
- Minimum of 3 years of nursing experience (hospice, home health or palliative care experience preferred)
- Current BLS certification required
- Valid driver’s license and reliable transportation required for home visits
- Able to respond within 45 minutes when on call
- Willingness to work holidays and participate in weekend coverage as scheduled
- Strong clinical assessment, communication, interpersonal and organizational skills
- Compassionate demeanor and ability to support patients and families through end-of-life care
Education
- Active Registered Nurse (RN) licensure in Utah or compact state
- Current BLS certification
Knowledge & Skills
- Patient assessment and care plan coordination
- Pain and symptom management in hospice settings
- Clinical documentation and use of hospice EMRs (Hospicesoft, Careficient preferred)
- Interdisciplinary collaboration with physicians, social workers, chaplains and volunteers
- Patient and family education on end-of-life care and self-care techniques
- On-call triage and rapid response skills for home-based hospice patients
