Candidate Pay Rate: $49 per hour

Roles & Responsibilities
- Perform comprehensive physical therapy evaluations to assess patient needs and progress
- Develop and implement individualized plans of care focused on functional outcomes and safe discharge
- Provide skilled treatments in patient rooms, hallways, outdoor areas, and the rehab gym
- Document timely and accurately using Cerner and NetHealth to ensure continuity of care
- Collaborate with the rehab team, participate in rounds and care conferences, and communicate patient progress
- Recommend and arrange appropriate equipment and support services to aid recovery
- Float between acute/swing, nursing home, and outpatient services as needed to support patient flow and continuity
Qualifications
- Current NC Physical Therapy license (required)
- CPR certification (required)
- Minimum 2 years of physical therapy experience (required)
- Experience in hospital and/or nursing home setting (must have)
- Preferred: 3 years of experience in a medical setting
- Comfortable documenting in Cerner and NetHealth (preferred)
- Strong interpersonal skills and ability to work in a collaborative rehab-focused environment
Knowledge & Skills
- Skilled in physical therapy evaluations and establishing plans of care
- Experience across rehab services: acute, swing bed, nursing home, and outpatient
- Familiarity with common rehab equipment and gym-based therapy
- Proficient documentation practices and electronic health record use (Cerner, NetHealth)
- Ability to communicate clearly with multidisciplinary teams and patients
- Comfort with 1:1 patient care and adapting treatment locations (room, gym, outdoor)
