Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in the assigned unit
• Administer medications and treatments accurately and safely according to orders
• Perform timely patient assessments and monitor clinical status, reporting changes promptly
• Document care and observations thoroughly in the patient record to support continuity
• Collaborate with nurses, therapists, and interdisciplinary team members to optimize outcomes
• Support quality and safety initiatives to maintain high standards of care
• Educate patients and families on care plans, medications, and discharge instructions

Qualifications

• Active Licensed Practical Nurse (LPN) license in New Mexico or eligibility to obtain one prior to start
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Comfort and experience working 12+ hour day shifts and scheduled weekend coverage
• Prior long-term care or rehab experience is a plus, travel LPN experience preferred
• Ability to complete facility onboarding and any required background checks or health screenings before start date

Education

• Graduate of an accredited Practical Nursing (LPN) program
• Completion of state-required LPN licensure requirements

Knowledge & Skills

• Strong medication administration and documentation skills
• Patient assessment and vital sign monitoring
• Wound care and basic restorative nursing skills
• Infection control practices and safety-focused care
• Effective communication and teamwork with interdisciplinary staff
• Familiarity with electronic health records and charting best practices
