Roles & Responsibilities

• Evaluate patients' functional abilities and create individualized occupational therapy plans
• Deliver evidence-based therapeutic interventions to improve daily living and work skills
• Collaborate closely with an interdisciplinary team to coordinate care and optimize outcomes
• Document progress, update treatment plans, and maintain timely charting to ensure continuity
• Educate patients and families on strategies and adaptive techniques to promote independence
• Participate in team meetings and discharge planning to support smooth transitions

Qualifications

• Licensed Occupational Therapist in New Hampshire or ability to obtain state licensure
• Current basic life support (BLS/CPR) certification preferred or willing to obtain
• Clinical experience in rehabilitation, outpatient, or post-acute care settings preferred
• Strong assessment, treatment planning, and documentation skills
• Excellent communication and teamwork; comfortable working with diverse patient populations
• Professional, reliable, and patient-centered approach to care

Education

• Entry-level degree in Occupational Therapy (e.g., Master's in OT or OTD) or equivalent

Knowledge & Skills

• Skilled in functional assessments, ADL/IADL training, and adaptive equipment recommendations
• Knowledge of evidence-based OT interventions and therapeutic activity progression
• Proficient documentation and electronic medical record usage
• Ability to collaborate with PT, SLP, nursing, and restorative teams
• Problem-solving mindset for tailoring interventions to individual goals
