Roles & Responsibilities

• Assess residents' speech, language, cognition and swallowing using clinical judgment and evidence-based tools
• Develop and implement individualized treatment plans to improve communication and swallowing function
• Provide skilled therapy, including dysphagia management and compensatory strategies, tailored to each resident
• Document evaluations, treatment plans and progress notes in a timely, compliant manner
• Collaborate with nursing, therapy colleagues and family members to support meaningful outcomes
• Educate residents and caregivers on strategies to generalize gains to daily life
• Participate in interdisciplinary meetings and contribute to discharge planning and continuity of care

Qualifications

• ASHA CCC-SLP (listed as primary requirement)
• 1-3 years of clinical SLP experience (SNF experience desirable)
• Active Tennessee licensure or eligibility to obtain state license
• Comfortable with some float coverage and adapting to different unit needs
• Strong documentation skills and ability to work as part of an interdisciplinary team

Knowledge & Skills

• Clinical expertise in adult speech, language, cognition and dysphagia management
• Skilled in assessment, treatment planning and progress documentation
• Ability to collaborate effectively with nursing and therapy teams
• Good communication skills for resident and family education
• Familiarity with SNF practice setting and regulatory documentation expectations
