Roles & Responsibilities
- Manage a caseload of approximately 15 hospice patients, providing compassionate, patient-centered care
- Conduct 4–5 patient visits per day (depending on geography and acuity) and complete same-day charting
- Perform admissions, SIA visits, and death visits with professionalism and sensitivity
- Deliver all service-level nursing care: routine, General Inpatient (GIP), continuous, and respite
- Coordinate care across the interdisciplinary team and communicate changes in patient status promptly
- Document thoroughly in the WellSky Consolo EMR to ensure continuity and regulatory compliance
- Travel within a 60-mile service radius to reach patients in their homes and community settings
- Maintain required screenings, vaccinations, and background/credential requirements to keep patients safe
Qualifications
- Active Washington RN license required (Compact license acceptable for submission)
- Oregon RN license strongly encouraged; application for OR licensure required upon accepting the contract
- Minimum 1+ years of current hospice case management experience
- Active American Heart Association BLS (required)
- Valid driver’s license and valid auto insurance; reliable vehicle to cover up to a 60-mile radius
- Clean background checks in WA and OR and successful drug screening
- Up-to-date vaccination records for Influenza, TB, and COVID
Education
- Nursing education leading to RN licensure (ADN, BSN or equivalent) — active RN license required
Knowledge & Skills
- Skilled in hospice nursing practices including routine, GIP, continuous, and respite care
- Proficient with clinical charting and same-day documentation
- Experience using WellSky Consolo EMR
- Strong assessment, clinical judgment, and care planning skills
- Good time management and geography-savvy routing for efficient field visits
- Comfortable performing admissions, SIA, and death visits with compassion
- Preferably some acute/medical-surgical background to support complex patient needs
