Roles & Responsibilities

• Provide compassionate, resident-centered direct care to support activities of daily living
• Assist with bathing, dressing, grooming, mobility, and feeding while preserving dignity
• Measure and document vital signs and report changes promptly to nursing staff
• Maintain accurate and timely documentation in the resident chart and EMR
• Communicate effectively with residents, families, and the interdisciplinary care team
• Support a safe, clean, and comfortable living environment and follow infection control practices
• Participate in care planning and team huddles to improve resident outcomes
• Respond quickly and professionally to resident needs and urgent situations

Qualifications

• Current Licensed Nursing Assistant (LNA) certification — VT or NH authorization noted in posting
• Active healthcare license and ability to provide proof of credentials upon onboarding
• Comfortable working the listed schedule including weekends
• Strong interpersonal skills and a patient-centered approach
• CPR/BLS certification preferred or willing to obtain

Education

• High school diploma or equivalent
• Completion of an approved LNA training program

Knowledge & Skills

• Knowledge of basic nursing procedures and resident care best practices
• Skilled at taking and recording vital signs accurately
• Proficient in documentation and basic electronic medical record entry
• Excellent communication and teamwork skills
• Ability to manage time, prioritize tasks, and remain calm under pressure
• Awareness of infection control and safety protocols
