Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Assess and treat patients with communication and swallowing disorders in home health settings
Develop and implement individualized care plans
Collaborate with interdisciplinary team for coordinated care
Document patient progress and maintain accurate records
Educate patients and families on therapeutic techniques
Maintain compliance with state and federal regulations

Qualifications

Active Kentucky SLP license (must be in hand upon submission)
Certificate of Clinical Competence (CCC-SLP)
Current BLS certification
Minimum 1 year clinical experience in home health
Ability to cover a large geographic territory (11 counties)

Knowledge & Skills

Strong assessment and diagnostic skills for speech and language disorders
Excellent communication and interpersonal skills
Knowledge of home health regulations and documentation
Proficient in electronic health records
Time management and organizational skills for busy caseload
