Location: Woodland, CA

End Date: 10-06-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM with occasional Saturday rotation (once per month)

Hours per Week: 40

Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Assess and evaluate patients' physical therapy needs in their home environment
Develop and implement individualized treatment plans to improve mobility and function
Instruct patients and caregivers on therapeutic exercises and equipment use
Collaborate with physicians and healthcare team to coordinate care
Document patient progress and submit clinical summaries accurately
Supervise home health aides in the patient's rehabilitation plan

Qualifications

Must have at least 2 years of experience as a Physical Therapist in an acute, home care, or rehab setting
Current BLS certification required
Valid Physical Therapist license in California (or ability to obtain)
Strong communication and organizational skills
Comfortable working independently in patients' homes

Knowledge & Skills

Physical therapy assessment and treatment techniques
Home health regulatory compliance (Title 22, Medicare, Medi-Cal)
Understanding of patient safety and infection control
Proficiency in electronic health records and documentation
Ability to adapt therapy plans to diverse home environments
