Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM; occasional Saturday rotation

Hours per Week: 40

Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Assess and evaluate patients' therapeutic and functional status in home health settings
Develop and implement individualized physical therapy plans of care
Instruct patients and families on therapeutic exercises and use of equipment
Collaborate with physicians and healthcare team to optimize patient outcomes
Document patient progress and maintain accurate clinical records
Supervise home health aides as needed

Qualifications

Minimum of 2 years experience as a Physical Therapist in acute, home care, or rehab setting
Current BLS certification
Valid Physical Therapy license in California
Strong communication and organizational skills
Comfortable traveling within Yolo County service area

Knowledge & Skills

Expertise in physical therapy assessment and treatment
Knowledge of home health regulations and Medicare guidelines
Ability to work independently and as part of a team
Excellent patient education and interpersonal skills
Proficiency in documentation and electronic health records
