Contract Type: Travel Contract

Shift Type: Monday - Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide skilled physical therapy services to patients in their homes
Develop and implement individualized treatment plans for post-surgery and fall recovery patients
Manage a caseload of 5-7 patients per day within a 60-mile radius
Collaborate with home health and hospice teams to ensure comprehensive care
Document patient progress in HomeCare/HomeBase EMR system

Qualifications

Active North Dakota Physical Therapy License
BLS certification required
Valid Driver's License and Auto Insurance
Minimum 1 year of physical therapy experience
Previous home health experience strongly preferred

Knowledge & Skills

Proficient in HomeCare/HomeBase EMR
Strong clinical assessment and treatment planning skills
Ability to travel up to 60 miles for patient visits
Excellent communication and patient education skills
