Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Provide comprehensive physical therapy services to patients in their homes across four counties
Conduct Oasis Assessments and Start of Care visits to develop individualized care plans
Help elderly patients recover from falls and fractures with personalized rehabilitation
Collaborate with interdisciplinary teams to ensure holistic patient care
Utilize a tablet for efficient time tracking and visit documentation

Qualifications

Active Physical Therapist (PT) license in Indiana
Experience with Oasis Assessments and Start of Care preferred
Comfortable working in rural and mid-sized town settings
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Proficient in Oasis documentation and home health regulations
Strong clinical assessment and care planning skills
Excellent communication and patient education abilities
Ability to manage a caseload of 4-6 patient visits per day
