Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct start of care assessments and develop individualized care plans for home health patients
Perform recertifications and resumption of care visits to ensure ongoing patient needs are met
Manage a daily caseload of 6.5 points, including regular visits and admissions
Provide patient and family education on home health management and safety
Collaborate with clinical coordinators and directors to optimize patient outcomes
Complete OASIS documentation accurately and timely

Qualifications

Active North Dakota Physical Therapy License required
BLS certification required
Valid driver's license and auto insurance required
Home health experience strongly preferred
OASIS training and experience preferred
Willingness to complete a manager interview - no clinical involvement required

Knowledge & Skills

Knowledge of home health regulations and OASIS documentation
Strong clinical assessment and decision-making skills
Ability to manage multiple visits and prioritize tasks effectively
Excellent communication and patient interaction skills
Comfortable working independently in the field
