Location: Carmichael, California

Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, with rotating weekends (at least one full weekend per month)

Hours per Week: 40

Candidate Pay Rate: $82 per hour

Roles & Responsibilities

Coordinate comprehensive discharge planning for patients across all hospital units
Perform utilization review using Interqual criteria to ensure appropriate level of care
Conduct initial assessments and develop individualized care plans
Collaborate with interdisciplinary team to ensure safe transitions of care
Advocate for patients, including uninsured and homeless populations
Manage a caseload of 15-20 patients per day, prioritizing effectively

Qualifications

Active California RN license required
Minimum 2 years of recent case management experience in an acute care setting
Prior experience with Dignity Health or CommonSpirit facilities is highly preferred
BLS certification from the American Heart Association
Proficiency with Interqual and Cerner electronic medical records
Strong communication, teamwork, and prioritization skills

Knowledge & Skills

Proficiency in utilization review and discharge planning processes
Knowledge of Interqual criteria and Cerner EMR
Excellent time management and prioritization abilities
Ability to work effectively in a fast-paced environment
