Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide physical therapy services to patients in a home health setting
Manage a caseload of 5-7 patients per day covering a 60-mile radius
Develop and implement individualized treatment plans for conditions such as surgeries and falls
Collaborate with interdisciplinary team members and document in HomeCare/HomeBase EMR
Conduct patient assessments and adjust care plans as needed

Qualifications

Current North Dakota Physical Therapy License
BLS certification
Valid Driver's License and Auto Insurance
Minimum 1 year of home health experience
Previous travel experience not required

Knowledge & Skills

Physical therapy clinical skills
Home health experience and patient management
Proficiency with HomeCare/HomeBase EMR
Strong assessment and treatment planning abilities
Excellent communication and time management skills
