Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Conduct comprehensive psychosocial assessments for patients and families
Develop and implement discharge plans to ensure safe transitions
Provide crisis intervention and support for behavioral health needs
Collaborate with interdisciplinary team members to coordinate care
Facilitate 5150 evaluations as required by California law
Advocate for patient rights and access to community resources

Qualifications

Current California LCSW license required
Current BLS certification required
Must be 5150 certified in California (completion of training and exam with 80% pass rate)
Prior experience with Dignity Health or CA CommonSpirit facilities strongly preferred
Minimum 1 year of acute care social work experience preferred

Education

Master's degree in Social Work from an accredited program

Knowledge & Skills

Thorough understanding of behavioral health and crisis intervention
Knowledge of California 5150 holds and psychiatric assessment
Strong communication and collaboration skills
Ability to manage a high caseload and prioritize effectively
Proficiency with electronic medical records
