Shift Type: Monday-Friday, Day shift

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct comprehensive patient assessments and develop individualized care plans
Perform start-of-care visits for orthopedic patients
Provide skilled physical therapy interventions in patients' homes
Collaborate with interdisciplinary team for coordinated care
Educate patients and families on therapeutic exercises and home safety
Document patient progress in HCHB EMR system

Qualifications

Valid Physical Therapist license in California
BLS certification required
Minimum 2 years of home health experience (less with home health experience considered)
Experience with HCHB EMR preferred
Ability to travel within the service area

Knowledge & Skills

Home health regulations and OASIS documentation
Orthopedic rehabilitation
Geriatric patient management
Excellent communication and interpersonal skills
Ability to work independently
