Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct start of care, resumption of care, and recertification visits for home health patients
Manage a caseload of approximately 6.5 points per day, including daily visits and admissions
Collaborate with clinical coordinators and sister agency therapists for support
Provide primarily home health services with occasional hospice safety evaluations
Maintain accurate and timely documentation of patient care
Ensure compliance with OASIS training and documentation standards

Qualifications

Current and active North Dakota Physical Therapy License
Current BLS certification
Valid Driver's License and Auto Insurance
Home health experience strongly preferred
OASIS training preferred
