Shift Type: Monday-Friday, 8:00 AM - 5:00 PM (flexible)

Hours per Week: 40

Candidate Pay Rate: $64 per hour

Roles & Responsibilities

Provide compassionate physical therapy services in patients' homes
Develop and implement individualized treatment plans to improve mobility and function
Collaborate with a multidisciplinary team including OTs, SLPs, and nursing staff
Complete OASIS documentation accurately and timely
Educate patients and families on home exercise programs and safety
Cover a designated geographical region with a focus on patient-centered care

Qualifications

Active Physical Therapist license in California
BLS certification required
Minimum one year of experience as a PT (home health experience preferred)
Experience with OASIS documentation mandatory
Strong communication and organizational skills

Knowledge & Skills

Proficiency in OASIS and home health regulations
Experience with Cerner EMR is a plus
Ability to work independently and manage a caseload
Knowledge of ortho, stroke, and spinal injury rehabilitation
