End Date: 09-12-2026

Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM (flexible), with every 3rd weekend

Hours per Week: 40

Candidate Pay Rate: $57 per hour

Roles & Responsibilities

Conduct comprehensive patient assessments and develop individualized care plans
Provide therapeutic interventions to improve patients' daily living skills
Collaborate with interdisciplinary team to ensure coordinated care
Educate patients and families on home exercise programs and adaptive techniques
Document patient progress using Oasis and Cerner EMR systems
Travel to patients' homes within assigned geographic region

Qualifications

Active California Occupational Therapist license required
BLS certification required (American Heart Association)
Minimum one year of experience as an Occupational Therapist
Home health experience preferred but not required
Experience with Oasis documentation required
Valid driver's license and reliable transportation

Knowledge & Skills

Knowledge of ortho, neuro, and geriatric conditions
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal abilities
Proficiency with EMR systems, especially Cerner
Ability to work independently and manage a caseload
