Roles & Responsibilities

• Perform skilled occupational therapy evaluations in patients' homes and develop individualized treatment plans
• Deliver meaningful interventions to improve patients’ functional independence and quality of life
• Educate patients and caregivers on safe home strategies, adaptive equipment, and energy conservation
• Coordinate care with interdisciplinary team members and communicate progress clearly
• Document thoroughly and timely in the electronic medical record to support continuity of care
• Adapt treatment approaches for diverse patient populations and home environments
• Manage a professional caseload while prioritizing patient safety and clinical outcomes

Qualifications

• Active Occupational Therapist license (or eligibility) to practice in Missouri
• Entry-level to experienced clinicians welcome — new grads encouraged to apply
• Comfortable with home health setting and frequent travel between patient homes
• Strong clinical reasoning, patient education, and documentation skills
• Valid driver’s license and reliable transportation for community visits
• Professional, compassionate approach and ability to work collaboratively

Education

• Degree in Occupational Therapy (e.g., MOT, MSOT, OTD) or equivalent entry-level OT education

Knowledge & Skills

• Skilled in home health assessments and functional goal-setting
• Knowledge of adaptive equipment, ADLs, and home safety interventions
• Excellent verbal and written communication for patient and team interactions
• Time management and caseload organization for efficient home visits
• Ability to adapt interventions to varied home environments and caregiver needs
