Roles & Responsibilities

• Provide skilled speech-language pathology evaluation and treatment to SNF residents
• Develop individualized treatment plans and measurable goals
• Document progress and maintain accurate clinical records per CRG policies and procedures
• Collaborate with interdisciplinary team members to support resident outcomes
• Meet productivity expectations, maintaining the facility requirement of 90% productivity
• Represent the therapy department professionally, following the facility dress code of black scrubs

Qualifications

• 1-3 years of SLP experience (as stated in the posting)
• Experience working in a skilled nursing facility or similar post-acute setting preferred
• Ability to follow Continuum Rehabilitation Group policies and productivity standards
• Reliable on-site availability to receive guaranteed hours
• Strong communication and teamwork skills to collaborate with rehab and nursing staff

Knowledge & Skills

• Skilled in assessment and treatment planning for SNF populations
• Proficient documentation skills and familiarity with facility/contract policies
• Ability to manage productivity and caseload expectations
• Team-oriented approach and strong resident advocacy
• Comfort working in a fast-paced rehab environment
