Roles & Responsibilities

• Provide safe, compassionate nursing care to patients in the Uptown Health/Rehab area
• Administer medications and treatments accurately and document care thoroughly
• Perform timely patient assessments and communicate changes to the care team
• Assist with rehabilitation therapies and support patient mobility and recovery
• Collaborate with interdisciplinary team members to implement individualized care plans
• Maintain accurate electronic and paper records, ensuring regulatory compliance

Qualifications

• Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license
• Current BLS (Basic Life Support) certification required
• Experience in rehabilitation or related clinical settings preferred
• Strong clinical judgment, attention to detail, and effective communication skills
• Ability to work the assigned night shift (6:00 PM - 6:30 AM) and deliver consistent care

Education

• Graduate of an accredited practical nursing program (for LPN) or nursing program (for RN)
• Completion of required state licensure program

Knowledge & Skills

• Skilled in medication administration and monitoring for adverse reactions
• Proficient in patient assessment, vital signs, and documentation
• Familiarity with rehab care routines and mobility assistance
• Comfortable collaborating with therapists and interdisciplinary teams
• Good time management and the ability to prioritize patient needs
• Basic proficiency with electronic medical record documentation
