Roles & Responsibilities

• Provide compassionate, skilled nursing care to patients in their homes — your care improves their daily lives
• Conduct thorough nursing assessments and develop individualized care plans
• Administer medications and treatments safely and accurately
• Document patient status and care activities in Cerner to ensure continuity and quality
• Coordinate with interdisciplinary team members and family caregivers for seamless care transitions
• Educate patients and families about condition management, medications, and follow-up care
• Monitor and report changes in patient condition promptly to ensure optimal outcomes

Qualifications

• Valid LPN/LVN license in good standing (License to submit: Yes)
• Minimum 2 years of nursing experience (as listed in the job details)
• Current BLS (Basic Life Support) certification required
• Comfortable with home health and post-acute care environments
• Experience or willingness to learn electronic charting in Cerner
• Able to complete a 12-hour orientation as part of onboarding

Education

• Vocational nursing diploma or equivalent leading to LPN/LVN licensure
• High school diploma or GED

Knowledge & Skills

• Home health nursing principles and post-acute care needs
• Skilled wound care and medication administration
• Patient assessment, care planning, and documentation best practices
• Care coordination and communication with interdisciplinary teams
• Proficiency (or ability to quickly learn) with Cerner electronic health records
• Strong patient and family education skills and cultural sensitivity
