Roles & Responsibilities

• Provide patient-centered occupational therapy evaluations and develop individualized treatment plans
• Deliver evidence-based OT interventions to improve daily living, mobility, and functional independence
• Document assessment findings and treatment progress accurately in EPIC and complete required Trinity checklists
• Collaborate with interdisciplinary teams across Livingston and Washtenaw Counties to coordinate care
• Educate patients and families on strategies to maximize independence and safety at home
• Participate in specialty competency testing and maintain clinical skills through ongoing training
• Ensure compliance with licensure, agency policies, and required health screenings

Qualifications

• Active Occupational Therapy license (state licensure verification required)
• BLS certification required
• Comfortable using EPIC for documentation and care coordination
• Completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• Ability to pass Trinity 10-panel drug screen, fit test, respirator evaluation, and required immunizations (COVID-19, MMR, Hep B, Influenza, Varicella, Tdap, PPD/IGRA)
• Professional, patient-focused approach with strong communication and teamwork skills

Knowledge & Skills

• Skilled in OT assessment, ADL retraining, and mobility interventions
• Proficient with EPIC documentation and clinical workflows
• Knowledge of outpatient and community-based rehabilitation best practices
• Strong clinical reasoning and goal-setting abilities
• Familiarity with infection control, safety screening, and respirator protocols
• Able to collaborate effectively with interdisciplinary teams across multiple sites
