Roles & Responsibilities

• Collaborate with the RN Care Manager (RNCM) to coordinate patient care and discharge planning
• Document assessments and care plans accurately and timely using Cerner and post-acute systems
• Communicate clearly with patients, families, and multidisciplinary teams to build trust and ensure continuity of care
• Engage patients and families in care decisions and provide emotional support and resources
• Prioritize and shift attention between tasks while managing a high-volume caseload
• Support patient experience initiatives and contribute to a culture of teamwork and professionalism

Qualifications

• Minimum 2 years of relevant experience (as listed in the job posting)
• Strong documentation and communication skills; comfortable using Cerner
• Experience with post-acute documentation systems (NATE, EnsoCare) is a plus
• Ability to build rapport and trust with patients, families, and care teams
• Ability to multi-task and float to different units as needed
• No specific certifications required for this posting
