Roles & Responsibilities
- Conduct comprehensive psychosocial assessments to identify patient strengths, needs, and risks
- Develop individualized treatment plans grounded in evidence-based therapeutic interventions
- Provide psychotherapy and brief interventions for a range of behavioral health concerns including depression, anxiety, trauma, and substance use
- Coordinate care with primary care providers, nurses, and behavioral health professionals to ensure whole-person, integrated care
- Document clinical encounters and treatment progress accurately and timely in the patient record
- Facilitate referrals and community resource linkage to support patients in medically underserved and rural settings
- Advocate for patients and support population health initiatives aimed at improving behavioral health access
Qualifications
- Master's degree in Social Work from an accredited program (MSW)
- Active LISW license in Iowa (required)
- Minimum of 2 years of clinical social work experience preferred
- Experience in rural or medically underserved settings is a plus
- Strong interpersonal, organizational, and clinical documentation skills
Education
- Master's degree in Social Work (MSW) from an accredited program
