Roles & Responsibilities

• Perform thorough physical therapy evaluations and establish individualized plans of care
• Deliver hands-on and gym-based treatments in patient rooms, hallways, outdoor areas, and the rehab gym
• Participate in care conferences and rounds to coordinate patient-centered discharge planning
• Communicate frequently with the rehab team about patient progress, equipment needs, and next steps
• Document treatments and progress accurately in Cerner and NetHealth systems
• Float between acute, swing, nursing home and outpatient services as needed to meet patient demand
• Maintain professional standards and contribute to a collaborative, patient-focused environment

Qualifications

• Current NC Physical Therapy license (required)
• CPR certification (required)
• Minimum 2 years of clinical experience; preference for clinicians with 3+ years in a medical setting
• Experience working in a hospital and/or nursing home setting (must have)
• Comfortable working with swing-bed and skilled nursing populations and outpatient care
• Ability to float across acute, swing, nursing home and outpatient services
• Proficiency with documentation systems such as Cerner and NetHealth preferred

Knowledge & Skills

• Physical therapy evaluations and plan of care development
• Rehab gym treatment techniques and typical rehab equipment
• Patient-centered documentation and outcome tracking in Cerner and NetHealth
• Experience with swing-bed, skilled nursing, acute care and outpatient therapy workflows
• Strong interdisciplinary communication and discharge planning skills
• Ability to manage 1:1 patient care and adapt treatment across varied environments
