Roles & Responsibilities

• Provide skilled physical therapy in patients' homes, delivering compassionate, patient-centered care
• Evaluate functional mobility and develop individualized treatment plans focused on safe independence
• Document thoroughly and accurately using HCHB to ensure continuity of care and compliance
• Collaborate with nurses, aides, and interdisciplinary team members to coordinate patient care
• Educate patients and caregivers on home exercise programs, safety, and fall prevention
• Manage a caseload of approximately 5-6 patients per day while prioritizing clinical outcomes
• Travel safely to patient homes—maintain a reliable vehicle for visits and time management

Qualifications

• Licensed Physical Therapist in North Carolina (required)
• Minimum 2 years of clinical experience; home health experience required for therapy roles
• Comfortable and experienced with HCHB documentation
• Strong clinical assessment and treatment planning skills in the home setting
• Valid driver’s license and reliable vehicle for patient visits
• Excellent communication and patient education skills

• Proficient with HCHB documentation system
• Home safety assessment and fall prevention strategies
• Skilled in mobility training, transfer techniques, and assistive device education
• Care planning and outcome-focused treatment approach
• Time management and caseload organization
• Strong interdisciplinary communication and patient/caregiver coaching
