Roles & Responsibilities

• Screen and evaluate patients to determine appropriate home health or hospice services
• Develop and periodically review individualized care plans with physicians, patients, and caregivers
• Coordinate patient care through clear interdisciplinary communication and case conferences
• Provide ongoing assessment of clinical, functional, and emotional status and document findings
• Communicate significant changes promptly to physicians and VNA colleagues
• Teach patients and caregivers self-care techniques, medication management, and health counseling
• Advise other disciplines of discharges and support smooth transitions of care

Qualifications

• Licensed Registered Nurse in Vermont and New Hampshire, or hold an unencumbered compact license
• Graduate of an accredited school of nursing
• Minimum 2 years of experience in Home Health and Hospice
• Comfortable working independently in the field while collaborating with an interdisciplinary team
• Strong documentation and communication skills
