Roles & Responsibilities

• Provide patient-centered physical therapy evaluation and treatment plans that improve mobility and function
• Deliver skilled interventions including therapeutic exercise, gait training, balance work, and functional mobility practice
• Document assessments, progress notes, and discharge summaries thoroughly and in a timely manner
• Collaborate with nurses, physicians, and rehabilitation team members to coordinate care and optimize outcomes
• Educate patients and families on home programs, adaptive strategies, and injury prevention
• Monitor and modify treatment plans based on patient progress and tolerance to interventions
• Participate in interdisciplinary meetings and contribute to continuous quality improvement efforts

Qualifications

• Licensed Physical Therapist in Pennsylvania or eligible to receive state license prior to start
• Graduation from an accredited physical therapy program (DPT preferred)
• Current Basic Life Support (BLS) certification or ability to obtain upon hire
• Comfortable working in a fast-paced rehabilitation environment and providing care across varied diagnoses
• Willingness to comply with credentialing, background checks, and drug screening required by facility

Education

• Graduate of an accredited Physical Therapy program (Doctor of Physical Therapy preferred)

Knowledge & Skills

• Strong patient assessment and clinical reasoning skills
• Experience with mobility training, therapeutic exercise, manual therapy, and modalities
• Proficient documentation skills and familiarity with electronic medical records
• Excellent communication and patient education abilities
• Ability to work collaboratively within an interdisciplinary team and adapt to varied caseloads
• Time management and organizational skills to manage schedules and caseload priorities
