Roles & Responsibilities
- Conduct thorough speech, language and swallowing assessments to identify patient needs
- Develop individualized treatment plans with measurable goals that improve communication and safety
- Deliver skilled therapy interventions across cognitive-communication and dysphagia areas
- Document progress and treatment outcomes clearly and timely in the facility electronic records
- Collaborate with nursing, PT/OT and families to ensure coordinated, patient-centered care
- Educate patients and caregivers on home strategies, adaptive equipment and discharge planning
- Participate in interdisciplinary team meetings and contribute to quality improvement efforts
Qualifications
- Active Pennsylvania license as a Speech-Language Pathologist required
- Master's degree in Speech-Language Pathology or equivalent professional credential
- ASHA Certificate of Clinical Competence (CCC-SLP) preferred or in progress
- Current CPR/BLS certification preferred
- Prior experience in skilled nursing, inpatient rehab or medical-surgical settings is a plus
- Strong documentation skills and familiarity with clinical outcome measures
Education
- Master's degree in Speech-Language Pathology (required)
Knowledge & Skills
- Skilled in evaluation and treatment of dysphagia and safe swallowing protocols
- Proficient with speech, language and cognitive-communication therapy approaches
- Strong goal writing, clinical reasoning and progress-tracking abilities
- Comfortable collaborating across disciplines and coaching caregivers
- Familiarity with electronic health record documentation and regulatory requirements
- Excellent verbal and written communication with patients and care teams
