Roles & Responsibilities
- Provide compassionate, skilled practical nursing care to residents at Mountain Ridge Center
- Administer medications and treatments accurately and document care in the patient record
- Assess resident conditions, report changes promptly, and collaborate with the care team to adjust plans
- Support safe transfers, mobility, and daily living activities while promoting resident independence
- Maintain clear, timely documentation and ensure compliance with facility policies and regulatory standards
- Participate in shift handoffs and team communication to ensure continuity of care
- Contribute to a positive, resident-centered environment through respectful and professional interactions
Qualifications
- Current LPN (Licensed Practical Nurse) license valid for practice in New Hampshire
- Completion of an accredited practical nursing program
- Basic Life Support (BLS) certification preferred or willingness to obtain upon hire
- Comfortable administering medications and performing routine clinical procedures
- Ability to pass background check, drug screen, and meet facility onboarding requirements
- Previous long-term care or related clinical experience preferred
Education
- Completion of an accredited practical nursing program (LPN diploma or certificate)
Knowledge & Skills
- Strong medication administration and documentation skills
- Familiarity with long-term care workflows and resident-centered care
- Good assessment and clinical observation abilities
- Clear communication and teamwork with interdisciplinary staff
- Time management and prioritization during busy shifts
- Basic proficiency with electronic health records or willingness to learn
