Roles & Responsibilities

• Perform thorough patient evaluations and develop individualized physical therapy plans
• Deliver high-quality home health PT services, including skilled interventions and clinical assessments
• Complete accurate OASIS documentation, recertifications, and required visit paperwork in a timely manner
• Manage visit scheduling and travel logistics to maximize patient access and productivity
• Maintain required productivity targets (27 points/week when under guarantee) while ensuring safe, effective care
• Provide clear communication with patients, families, physicians, and interdisciplinary team members
• Adhere to infection-control protocols and safely treat COVID+ and PUI patients as needed
• Document visits thoroughly, including phone calls and care coordination activities

Qualifications

• Current CA Physical Therapy License (required)
• BLS (Basic Life Support) certification
• Valid driver’s license, current auto registration, and auto insurance
• Up-to-date immunizations and screenings: TB (annual), MMR, Varicella, HepB, TDap (within 10 years), annual Flu
• Negative drug screen and current physical exam per onboarding requirements
• Ability and willingness to travel to patient homes across assigned territory and occasionally beyond
• Comfortable providing care to COVID+ and PUI patients when required
• Strong time management and ability to meet productivity expectations

Knowledge & Skills

• Proficiency with OASIS documentation and home health visit types (SoC, Recert, Discharge, etc.)
• Skilled in patient evaluation, clinical assessments, and visit documentation
• Familiarity with per-visit billing concepts and productivity-to-hours conversion
• Excellent patient-centered communication and care coordination skills
• Strong organizational skills for scheduling and managing a mobile caseload
• Ability to work independently in the field and make sound clinical decisions
