Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications and treatments accurately and on schedule
• Monitor and document vital signs, patient status, and care notes in the chart
• Coordinate with RNs, physicians, and interdisciplinary team members for patient-focused care
• Perform wound care, IV site checks, and other assigned treatments as needed
• Educate patients and families on care plans, medications, and discharge instructions
• Support safe transfers, mobility, and fall prevention for residents
• Participate in shift handoffs and communicate changes in condition promptly

Qualifications

• Active LPN license in the US or ability to obtain New Hampshire licensure quickly
• Current BLS/CPR certification required
• Experience in long-term care, skilled nursing, or medical-surgical settings is a strong plus
• Ability to pass background check and drug screen
• Reliable, professional, and committed to high quality patient care

Education

• Completed Practical Nursing program or LPN diploma/certificate
• Any additional certifications or continuing education in nursing care are welcome

Knowledge & Skills

• Strong medication administration and documentation skills
• Comfortable with routine nursing procedures and infection control practices
• Proficient with electronic health records and timely charting
• Excellent communication and bedside manners with patients and families
• Time management and prioritization in a fast paced environment
• Team oriented with a collaborative approach to patient care
