Roles & Responsibilities

• Deliver occupational therapy interventions under the direction of the therapy team to improve resident function
• Assist residents with activities of daily living and adaptive strategies to increase independence
• Collaborate with nurses, therapists, and care staff to create and implement individualized care plans
• Document resident progress, interventions, and outcomes clearly and timely
• Educate residents and families on strategies, adaptive equipment, and home programs to support carryover
• Support a safe and therapeutic environment by following facility policies and infection control practices
• Participate in team meetings and communicate care updates to ensure coordinated services

Qualifications

• Certified Occupational Therapy Assistant (COTA) credential
• Eligible to work in Oregon and meet state requirements for COTA practice
• Comfortable providing care in a Skilled Nursing Facility (SNF) setting; new grads are welcome
• Strong communication and teamwork skills to collaborate with interdisciplinary staff
• Ability to document accurately and meet regulatory documentation standards

Education

• Associate degree or diploma in Occupational Therapy Assistant or equivalent (typical requirement for COTA)

Knowledge & Skills

• Knowledge of occupational therapy principles and interventions for adult and geriatric populations
• Skill in ADL retraining, adaptive equipment recommendations, and functional mobility support
• Proficiency with clinical documentation and care plan implementation
• Ability to assess functional status and report measurable progress
• Teamwork mindset and good interpersonal skills to engage residents and families
