Roles & Responsibilities

• Assess patients in their homes to create individualized, functional therapy plans
• Deliver skilled physical therapy treatments focused on mobility, safety, and independence
• Document patient progress clearly and timely to ensure continuity of care
• Collaborate with nursing and interdisciplinary teams to coordinate patient-centered care
• Educate patients and caregivers on home exercise programs, fall prevention, and safe transfers
• Adapt care plans as patients progress and communicate changes with the care team

Qualifications

• PT or DPT (Physical Therapist) – licensure/credentials appropriate for practice
• Open to all experience levels — new graduates are welcome to apply
• Comfortable practicing in a home health setting with frequent floating between clients
• Professional, patient-centered attitude with strong communication skills

Education

• DPT or equivalent Physical Therapy degree preferred; new grads accepted

Knowledge & Skills

• Skilled in physical therapy evaluation and treatment in home health environments
• Strong documentation and clinical decision-making abilities
• Ability to build rapport with patients and caregivers in their homes
• Flexible problem-solving when managing varied home setups and needs
